Licensed by the Arizona Department of Health Services, Weyrich Health Care Center is Medicare-certified and provides short- and long-term skilled nursing care on a 24-hour basis. Every resident is admitted and discharged to the nursing center by his or her own personal physician. Upon admission, each person is evaluated by a team of highly qualified, geriatric health professionals, which includes a physician, licensed nurse, social worker, therapeutic recreation specialist, dietitian, and rehabilitation therapist. Together, this team designs a plan of care that will meet the physical, emotional, social, and spiritual needs of your loved one. We believe families are one of the most important members of our team, and we highly encourage you to participate in this process.

Consultants are another important part of our health services team. We offer a variety of consulting services to meet the needs of our residents. These include rehabilitative therapies, psychology, dentistry, podiatry, pharmacy, ophthalmology, radiology and laboratory services. Our rehabilitative services include physical, occupational, and speech therapies. Our goal is to help our residents regain and maintain their highest level of functioning. To accomplish this, each rehabilitative program is designed specifically for the resident by our team of professionals. Helping our residents maintain an active lifestyle is as important to their well-being as providing quality medical care.

Our therapeutic recreation specialist works closely with all Residents to develop a stimulating program for each individual. Our programs and activities are specifically designed to enhance self-esteem and encourage individual expression. Residents are encouraged to participate in activities which are best suited to their abilities, interests, and personalities. At times, people even rediscover talents which have been dormant for years.

Special group outings are also organized. Our wheelchair accessible van takes interested Residents to special areas and activities, including drives to view the beautiful lakes and parks in nearby areas. Situated at the hub of the Health Care Center is a comfortable living room with a large screen television, equipped with a VCR to view favorite movies.

Dining Services: Residents are served three meals each day in our attractive dining room. All meals are planned under the direction of our consulting registered dietitian. Our dietary manager is also available to discuss any dietary concerns or personal food preferences with our residents and their families.

We know how important it is to have friends and family visit. Guest reservations can be made with our dietary staff if you wish to eat in the Weyrich Center dining room or reserve the private dining room for birthday parties and other special events.

Beauty/Barber Shop:A full service beauty/barber salon is located within our Health Care Center and is open five days a week. Manicures and pedicures are also available.

Religious Services: A non-denominational chapel is located within Westminster Village and features a service, performed by visiting area clergy, every Sunday at 4 p.m. A Jewish service is also held each month. For the convenience of our residents, a religious service is conducted every Friday afternoon in the Health Care Center by our Resident chaplain. Our chaplain is also available for personal counseling and makes regularly scheduled visits to see residents in our Health Care Center and local hospitals.

Personal Laundry Service: Weyrich Health Care Center encourages all residents to dress in casual clothing. We believe this is an important part of wellness. We do provide laundry service for an additional monthly charge.

Our staff has been specifically trained to help maximize each resident’s highest level of functioning by identifying and building upon each person’s strengths and capabilities. Family members are encouraged to be involved with their loved ones through these programs and family meetings.

Westminster Village Home Health Services is licensed as a non-Medicare participating home health agency by the Arizona Department of Health Services. A full-time R.N. Director supervises the staff and a Medical Director and Professional Advisory Group oversee the program.  The office is located on the second floor near Saguaro Lounge and is open Monday through Friday from 8 a.m. to 5 p.m. However, home health services are provided 7 days a week from 6:30 a.m. to 9:30 p.m. daily.

Home care services are designed to bridge the gap in services for Residents not yet ready to move to the next level of care.  A physician’s order is necessary for both nursing and personal care services.

Staff members assist Residents with activities of daily living such as bathing, grooming, dressing, meal preparation, medication reminders and wheelchair transportation.  These needs may arise during an acute episode of illness or when a chronic illness or disability prevents a Resident from performing such activities.

All services MUST be delivered in the Resident’s apartment according to State law. The need for services is evaluated by a Registered Nurse during an initial assessment visit.  Home health aides/homemaker are assigned duties and supervised by the R.N. staff regularly.  The following services are offered:

  • Skilled Nursing Care & Case Management
  • Home Health Aide/Homemaker
    • Personal Care: Bathing, grooming, dressing, medication reminders.
    • Supportive Care: Wheelchair escorts, light housekeeping, meal preparation, well-being checks, and physician visit escorts.

Other functions of the program include maintaining the Resident’s personal health information and offering various health and wellness programs to the community such as the annual flu shot clinic and fall prevention seminars.

Home Health Fees: The program is a fee-for-service, private pay agency and does not bill Medicare. A complete list of current charges is available from the home health office.  All tasks are assigned and billed a minimum time allotment. For example, shower assistance is alloted 30 minutes.

We provide up to thirty (30) minutes per day of home health aide/homemaker services to eligible Residents at no additional charge based on a mutually developed plan of care.  Care extending past 30 minutes is billed at the rate of $18 per hour.

  • Home Health Aide Personal Care Services: These services are rendered by certified nursing assistants and are charged at the rate of $18 per hour.
  • Non-Medical Supportive Services Assistance is provided by trained homemakers and home health aides and is charged at the rate of $18 per hour.
  • Nursing Services: A Registered Nurse is available seven (7) days a week for direct care and case management.  The nursing fee is $40 up to one hour. Each additional 15 minutes thereafter is billed at $ 10. Frequently used services, such as injections, wound dressings, and filling weekly medication boxes are billed at $20.  The charge for instillation of eye or ear drops is $10. There is no charge for emergency triage visits to residents.
